List of documents required for registration as a medical practitioner, pharmacist or optician

All documents submitted must be in the English language. Foreign language documents MUST be translated into the English language, be certified by the Translator and then sealed by a notary public. All copies must be certified and sealed by a notary public.

Documents to be submitted:

  • Letter addressed to the Registrar requesting registration
  • Oath in support of the Medical Registration Form is to be completed and witnessed before a notary.
  • Two professional references
  • A Letter of Good Standing from the Medical Board or Medical Registrat from the last jurisdiction worked.
  • Current relevant practice certificate/licence
  • Notarized copies of all degrees, diplomas and ceritficates.
  • Notarized copies of Passport identify pages
  • Current original police certificate
  • Your current email address, local address and telephone number.
  • 3 x passport size photographs
  • Current curriculum vitae

The words to be used as the certificate

For foreign translation:

"It is hereby certified that the within copy has been compared by me with the original and I find it to be true and complete copy."

For copy document:

"True and complete copy of the Original shown to me"
